Group Health Insurance: Carrier vs. CCSB vs. CalChoice
When a small business shops for employee health coverage, it helps to compare how coverage is offered before choosing a carrier. You can select a carrier’s small-group plans, use Covered California for Small Business (CCSB), or consider CaliforniaChoice (CalChoice). With any of these approaches, compare the actual plans, premiums, employee choices, eligibility rules, and administration before deciding.
This guide explains what to compare as you prepare for 2027 enrollment or renewal. It does not mean that all 2027 premiums or participating carriers have been finalized. Before making a decision, check the official program materials and carrier proposals for the applicable plan year.
Start with your business and workforce
List your W-2 employees, full-time equivalent (FTE) count, each employee’s hours and work location, and current coverage. Whether the business qualifies for a group plan and which employees can be offered coverage are separate questions. Choosing carrier plans directly
With this approach, the business compares small-group products from insurance carriers and decides which plans to offer. A broker can help, so “direct carrier” does not mean enrolling without an advisor. It also does not necessarily mean offering just one plan from one carrier. Available plan combinations depend on the carrier and contract terms.
This approach may be worth considering when employees generally use the same provider network and the business wants a manageable set of proposals, bills, and renewals. But direct carrier coverage is not automatically less expensive, and it does not automatically eliminate employee choice. Compare the actual quotes and plan designs.
Covered California for Small Business (CCSB)
CCSB is California’s health and dental insurance marketplace for small businesses. Employers set the coverage level and contribution, while employees can compare the carriers, plans, and provider networks available within the offered choices. Offering choices from multiple carriers is not unique to CalChoice; CCSB can offer that flexibility too. Enrollment, billing, and employee changes can be managed through MyCCSB.
Some eligible businesses that enroll through CCSB may qualify for the federal small-business health care tax credit. The credit is not automatic simply because you use CCSB; review employee count, average wages, employer contributions, and other requirements with a tax professional. CaliforniaChoice (CalChoice)
CaliforniaChoice is a private small-business benefits program that lets employers compare plans from participating carriers and offer employees a choice. The employer sets its contribution, and employees select from the plans available to them. Employees may pay the difference if the premium for their chosen plan exceeds the employer contribution.
Although the names and structures can sound similar, CaliforniaChoice and CCSB are different programs. CaliforniaChoice has its own business and employee eligibility, participation, and employer-contribution rules. Do not assume that the federal SHOP tax credit considered for qualifying CCSB enrollment also applies to CalChoice enrollment. Read our CaliforniaChoice program guide for details.
Which approach fits your business?
Instead of choosing based on the program name, request quotes using the same business information for each available approach.
- Employee choice: Which carriers, plans, and provider networks can employees actually choose?
- Total cost: What are the monthly premiums for employees and dependents, the employer and employee shares, and the deductibles?
- Care access: How does each plan cover the doctors, hospitals, and prescriptions employees use?
- Enrollment rules: What are the business eligibility, employee participation, contribution, and other-coverage rules?
- Administration: Who manages new hires, departures, invoices, renewals, and employee questions?
Use the same employee census and coverage start date to make the comparison meaningful. Carrier and plan availability and program rules can change, so confirm your final choice against current proposals and official documents.
