What is IRMAA?
IRMAA stands for Income-Related Monthly Adjustment Amount: an extra federal charge for Medicare Part B and Part D, separate from your plan premium. Social Security generally uses your 2024 tax return for 2026. California follows the same federal brackets.

How do I pay IRMAA?
Estimate only. Social Security makes the official determination. If your income fell because of a qualifying life-changing event, review the SSA-44 process before assuming your current income changes the amount.

A lower-IRMAA request and an appeal are different official processes. Start with the reason you disagree with the Social Security notice, then choose the matching path below. Do not send tax returns, Medicare numbers, or Social Security numbers to Skyline Benefit.
Income changed after a life event
Request a lower IRMAA with Form SSA-44.
A qualifying event, such as retirement through work stoppage, may allow a new initial determination using more recent income. SSA-44 also explains how to report anticipated reductions. Approval is not automatic.
- Examples: work stoppage or reduction, marriage, divorce, death of a spouse, loss of pension income, or certain employer settlement payments.
- Prepare your notice, more recent MAGI information, and evidence of the life-changing event.
The notice or determination is wrong
Request reconsideration with Form SSA-561.
Use this appeal path when you disagree with Social Security’s decision itself, such as the information used or the determination shown on the notice.
- Follow the deadline in your notice. Reconsideration is generally due within 60 days after you receive the determination notice, not simply 60 days after the decision date.
- State the issue, explain why you disagree, and provide the supporting records Social Security requests.

What could a lower tier change?
If Social Security moves one person with Part B and Part D from the first 2026 IRMAA tier to the standard tier, Part B falls from $284.10 to $202.90 and the $14.50 Part D adjustment is removed. That is $95.70 less per month, or $1,148.40 over 12 months if the reduction applies for the full year. The Part D plan premium is still separate.
This is hypothetical, not a client result. Your decision, effective date, and any refund depend on Social Security. Each spouse must request a new determination separately.

Not every income drop is an SSA-44 event.
A voluntary property sale or a Roth conversion is not itself a qualifying life-changing event. If you amended a tax return or the tax information is incorrect, contact Social Security about the appropriate correction or new-determination process.

The standard 2026 Part B premium is $202.90 per month. People subject to IRMAA pay a total Part B amount from $284.10 through $689.90 per month, depending on filing status and 2024 modified adjusted gross income. Social Security makes the official determination.
How much is the 2026 Part D IRMAA?
The 2026 Part D income-related adjustment ranges from $14.50 through $91.00 per month. It is paid in addition to the prescription drug plan premium and is generally collected by Social Security or Medicare rather than the plan.
What is Medicare IRMAA?
IRMAA is an additional amount that some people pay with their Medicare Part B and Part D premiums when Social Security determines that their modified adjusted gross income is above the applicable threshold.
What income does Social Security use for IRMAA?
Social Security generally uses tax information from two years earlier. For IRMAA, modified adjusted gross income generally means adjusted gross income plus certain tax-exempt interest. Your determination notice identifies the tax year and information used.
Can I ask Social Security to lower my IRMAA?
You may request a new determination after a qualifying life-changing event that reduced household income, or challenge incorrect tax information. Social Security provides Form SSA-44 for qualifying life-changing events and separate reconsideration procedures.
Will changing Medicare plans remove IRMAA?
No. IRMAA is determined by Social Security and is separate from a Medicare Advantage or Part D plan premium. Changing plans does not by itself remove the adjustment.
Can retirement qualify me for a lower IRMAA?
Retirement may qualify as work stoppage, or reduced hours as work reduction, when the event lowers the income used for IRMAA. Form SSA-44 is one way to request a new initial determination. Approval is not automatic; Social Security reviews the event, income, filing status, and evidence.
Are IRMAA brackets different in California?
No. IRMAA is a federal Medicare adjustment, so California residents use the same applicable income brackets as other U.S. residents. Skyline Benefit offers Medicare plan guidance in California; Social Security decides IRMAA requests.
Does an approved IRMAA request automatically cover both spouses?
No. Although married joint filers use household MAGI, IRMAA is charged per person. Each spouse who wants a new determination must make a separate request to Social Security.
