Eligibility is governed by the employer’s plan documents, policy, and carrier requirements. Confirm the employee’s hire date, classification, waiting period, and enrollment deadline.

Can an employee change coverage after a life event?

A qualifying event may permit a midyear change, but the plan’s rules, documentation requirements, and deadline control the result.

Who should handle a leave of absence?

Coordinate HR, payroll, the carrier or TPA, and the plan documents. Leave rules can have different requirements from ordinary active-employment changes.

What should be documented for a termination?
