PPO and Select PPO plans support broader access, while Virtual Access Plus plans combine virtual-first care with PPO coverage.
Most EmployeeElect plans use the RxChoice tiered pharmacy network. SHOP mirror plans may use a different pharmacy network.
Plan designs span Platinum, Gold, Silver, and Bronze, including deductible and HSA-compatible options.
Eligibility and submission
Anthem reviews the employer structure, payroll documentation, employee eligibility, and plan elections before approval.
Current wage or payroll reports and ownership documentation are commonly required to establish an eligible California small group.
Employee and dependent eligibility, active-work status, waivers, and special owner situations must be documented.
The employer contribution and employee participation are evaluated against current small-group requirements and valid waiver rules.
